Not strictly necessary, and does not fix any bug. This just corrects a
nitpick that technically audio encoder packets are start points, but
that they aren't being labeled accordingly as keyframes.
The empty clause was originally introduced to set a cutoff settings as a
hack at a commit ae862c16a and later removed at a commit a89470d2e. At
this time, the clause became empty but another commit aa58b9cf5 reuse
the clause and finally a commit 9baedb1c6 removes so that the clause is
empty now.
This reverts commit aa58b9cf5f.
FFmpeg has reverted their default AAC encoder from fast to twoloop,
which has much better rate control management, making it closer to CBR,
and it sounds much better.
The aac spec has a list of default channel layouts that are implemented
by all aac encoders / decoders.
Unfortunately 2.1 is not in that list. When I wrote the surround sound
support into OBS, as a workaround I decided to encode 2.1 as
AV_CH_LAYOUT_SURROUND = FL+FR+FC.
The LFE channel is encoded as a regular audio channel which uses more
bits but this allows compatibility with all aac decoders.
Recently we updated to the new channel layout API of FFmpeg, but the
mapping of 2.1 to AV_CH_LAYOUT_SURROUND was forgotten in the FFmpeg
native aac encoder. This is remedied here.

This fixes deprecation warnings since the channels member of
AVCodecContext is marked as deprecated [1], as well as the channels
member of AVFrame [2].
In all instances where a warning appear, a switch to the new API is
done.

The channel_layout API was overhauled by FFmpeg [1-4]. The previous
bitmask channel_layout is replaced by a struct ch_layout which combines
the number of channels, a bitmask and other infos. This struct must now
be supplied to AVframes since avutil >= 57.24.100 and to
AVCodecContext since avcodec 59.24.100 per (1].
This commit provides the required info to ffmpeg-mux,
obs-ffmpeg-output & to obs-ffmpeg-audio-encoders.

avcodec.h stopped including channel_layout.h per FFmpeg commit
1be3d8a0cb77 [1]. avformat.h stopped including avcodec.h per FFmpeg
commit e67e02d15672 [2]. As a result, we need to explicitly include
avutil/channel_layout.h when needed. Fixes compilation error against
FFmpeg later than the two mentioned commits.

Code submissions have continually suffered from formatting
inconsistencies that constantly have to be addressed. Using
clang-format simplifies this by making code formatting more consistent,
and allows automation of the code formatting so that maintainers can
focus more on the code itself instead of code formatting.
After you call av_frame_alloc(), ffmpeg expects you to fill in certain
fields on the frame, depending on whether it's an audio or video frame.
obs-ffmpeg did this in the two places where it allocates video frames,
but not where it allocates audio frames. On my system, using trunk
ffmpeg and the Opus codec, this causes OBS to crash while calling
avcodec_send_frame, ultimately because av_frame_copy fails due to
'dst->format < 0' (as 'format' stays at the default of -1), causing a
null pointer to be added to a buffer queue, which later gets
dereferenced.
Oddly, the fields in question can just be copied directly from
corresponding fields in the AVCodecContext, but I don't see any ffmpeg
API to automatically copy all relevant fields, and all the examples I've
seen do it by hand. So this patch does the same.
The cutoff hack was added many, many years ago as recommended by
Konverter. Since then, there has been much work on the AAC encoder, so
this hack should no longer be necessary.

The default channel layouts from aac spec are implemented in FFmpeg
native aac encoder as follows:
AV_CH_LAYOUT_MONO,

The correspondence of speaker layouts to AV_CH_LAYOUT from FFmpeg is
changed to reflect the previous table.
Although FFmpeg native aac encoder can now encode all the layouts listed
in avutil channel_layout.h (on master), there might be issues with older
FFmpeg binaries.
Note that 2.1 speaker layout will be encoded as AV_CH_LAYOUT_SURROUND
(FL FR FC) because it is not listed as the default layout for three
channels.
This just means some optimizations for LFE channel will not be used by
the encoder which will treat it as an SCE (single channel element).

Default channel layout for 4 channels is 4.0 in FFmpeg.
Replacing quad with 4.0 will improve compatibility since FFmpeg has
better support of its default channel layouts.

Removes speaker layouts which are not exposed in UI. The speaker
layouts selectable by users in the UI are the most common ones. It is
not necessary to keep other layouts. (This basically removes
5POINT1_SURROUND, 7POINT1_SURROUND, SURROUND =3.0).
